The Global Consensus Position Statement on the Use of Testosterone Therapy for Women— endorsed by major international societies including The International Menopause Society, The North American Menopause Society, The Endocrine Society, and others- concluded that testosterone therapy, in doses that approximate physiological testosterone concentrations for premenopausal women, exerts a beneficial effect on sexual function including increases, above the effects of placebo/comparator therapy, in the number of satisfying sexual events per month, and increases in the subdomains of sexual desire, arousal, orgasmic function, pleasure, and sexual responsiveness.

In the research they assessed, some commonly monitored androgenic effects, such as acne or mild increases in body/facial hair, were observed occasionally, but there was no consistent evidence of more pronounced androgenic effects, such as permanent voice changes or clitoromegaly, at physiologic doses.

Also, importantly, current evidence does not show any increased risk of breast cancer, and some data suggest it may even play a protective role in breast tissue.

That said, testosterone can also be delivered through several alternative routes, each with its own advantages and considerations:

  • Topical creams or gels are applied daily and allow for dose flexibility. However, absorption can be variable, levels may fluctuate, and there is a risk of transference to partners or children if precautions are not followed.
  • Transdermal patches provide consistent daily dosing but may cause skin irritation and are less commonly used due to tolerability issues.
  • Injectable testosterone (typically intramuscular or subcutaneous) can be effective but may create higher peaks and lower troughs between doses, which some women experience as mood or energy swings unless dosing is carefully managed.
  • Oral formulations are generally not recommended for women due to their potential negative effects on lipid profiles and liver metabolism.

Regardless of the method chosen, the most important factors remain physiologic dosing, regular monitoring, and personalized care to ensure both safety and satisfaction.
